CHURCH VEHICLE REQUEST FORM

 

All requests must be submitted to the Vice-Chairperson of the Trustee Ministry at least two (2) weeks prior to the date the vehicle is needed.  In situations where it is not possible to submit this form two (2) weeks in advance, the Vice-Chairperson of the Trustee Ministry will use good judgement in trying to accommodate the requestor.  If the Vice-Chairperson decides not to honor the request, the Pastor should immediately be consulted for a final decision.

The Vehicles are limited to 15 passengers including the driver.  There are absolutely NO EXCEPTIONS.  When filling out the passenger total field on the form, be sure to include the driver in your total,

An approved driver must be secured by the requestor before submitting the request.

Upon return of the vehicle, the actual Leave and Return times must be given to the Trustee Chairperson or Vice-Chairperson for documentation purposes.
